The National Alliance on Mental Illness of Waukesha County is the lead agency for the Waukesha County Projects for Assistance in Transition from Homelessness (PATH) that provides services for individuals experiencing homelessness who also have a serious and persistent mental illness or a co-occurring illness (mental illness coupled with a substance abuse disorder.)
The Waukesha County PATH Team is made up of an Outreach Coordinator employed with Hebron House of Hospitality, a Program Coordinator, four Certified Peer Specialists (individuals with lived experience) and a Coordinator of Homeless Outreach and Veteran Services employed by NAMI Waukeshaand a Nurse Case Manager employed by St. Joseph's Medical Clinic. The team works collaboratively to provide active outreach in the community to identify individuals who are literally homeless or in imminent danger of becoming homeless and to connect them with the support and services needed to find and maintain safe, affordable and permanent housing.
The PATH Team provides street outreach on a weekly basis by visiting areas where people experiencing homelessness are known to camp, including those places not meant for human habitation. We are in need of items to be used for street outreach during the winter months.

- Canned foods with pull tabs
- Bottled Water
- Batteries
